Complete the Consumer Report User Agreement

Download the Agreement or complete online
Print the form
Sign the form
Return the completed form to Premier
via fax or mail

Immediately begin asking your applicants to complete Premier’s Release and Disclosure Authorization form.

* Be sure to provide each applicant with a copy of the FCRA Summary of Rights. CA residents should also receive a copy of the California FCRA Summary of Rights.

Your account will be activated in 48 hours or less, Mon – Fri based on the time your completed application is received by Premier. Upon activation, one of our Customer Care Representatives will contact you to provide the following:

An activated account and User ID
Personalized instructions for accessing Premier’s on-line order entry system or ordering via fax
Detailed product specifications and requirements
Answers to questions you may have as you are getting started
